The Cambridge-Guernsey County Health Department partners with Access Tusc to offer Bridges
to Wellness, a program that connects families to local resources to help them become stronger
and healthier. Through Bridges to Wellness, families can access all the services they need in
one place by working with an experienced community care coordinator who will come to their
homes, listen to their needs, and work with them to create a plan to make sure their needs are
met.
Who can benefit from Bridges to Wellness?
All pregnant women, as well as men, women and children of Guernsey County who need
connected to local services and support.
How does the program work?
A community care coordinator from the Cambridge-Guernsey County Health Department will
meet with you to determine your needs (i.e. transportation, food assistance, clothing assistance,
medical providers, education, housing, etc) and then work with you to develop a plan to meet
these needs. Additionally, education and support are provided to pregnant women to help
promote a healthy pregnancy and post-partum period.
